Integrating TCM for Fertility Treatment Support

IVF & IUI in Singapore: TCM & Natural Support

Many women and couples reach a point in their fertility journey where they start considering IVF or IUI. It may come after months of trying naturally, or after being told that ovulation, egg quality, sperm quality, or hormonal balance needs support. It is very normal to feel uncertain, hopeful, and anxious at the same time.

At Wo.men Matters, we believe that fertility support should be gentle, personalised, and rooted in understanding the body as a whole. Whether you are preparing for IVF, exploring IUI, or trying naturally, there are ways to support your reproductive health with Traditional Chinese Medicine (TCM) alongside medical care.

WHAT

IVF vs IUI in Singapore: Which Treatment Is Right for You?

Intrauterine insemination (IUI) and in vitro fertilisation (IVF) are the two main assisted reproductive technologies available in Singapore.

IUI is a treatment where sperm is washed and placed directly into the uterus around the time of ovulation. It is often used when:

  • Sperm count or movement needs gentle support
  • Ovulation is regular
  • Couples experience unexplained infertility
  • Timing conception has been difficult

It is less invasive, has a lower cost, and is often a first step before IVF.

IVF (In Vitro Fertilisation) is a more complex process which involves retrieving eggs, fertilising them in a lab, and transferring the embryo into the uterus. IVF is often recommended when:

  • IUI attempts have not worked
  • There are fallopian tube blockages
  • There are more pronounced sperm or egg quality issues
  • There are known reproductive conditions such as endometriosis
  • Older couples (particularly if the wife is older than 37 years old) who are aiming for a family of two or more children
Neither treatment is inherently “better.” The appropriate choice depends on your specific circumstances, diagnosis, age, and previous fertility history.

IUI advantages:

  • Less invasive with minimal discomfort
  • Lower cost compared to an IVF cycle
  • Fewer medications and side effects
  • Can be tried multiple cycles with minimal risk
  • More natural process
IVF advantages:
  • Higher success rates per cycle
  • Bypasses multiple fertility obstacles simultaneously
  • Allows embryo genetic testing (limited circumstances)
  • Can freeze embryos for future pregnancies
  • More control over process and timing

IUI in Singapore: Step-by-Step Process

Understanding what IUI involves helps reduce anxiety about the unknown and allows you to prepare physically and emotionally for treatment.

Step-by-step IUI process

  1. Tracking your ovulation window
  2. Ovarian stimulation
  3. Trigger medication (if needed) to time egg release
  4. Sperm washing and preparation
  5. Sperm placement into the uterus
  6. Two-week wait for pregnancy testing

How long does it take to get pregnant with IUI?

IUI is typically approached as a multiple-cycle treatment, with most pregnancies occurring within the first 3-4 attempts if the procedure is going to work.

IVF in Singapore: Step-by-Step Process

IVF is more complex than IUI, involving multiple stages over approximately 4 weeks from start to pregnancy test, unless a suppression shot was given 2 weeks before the stimulation begins. Understanding each phase helps you prepare.

Step-by-step IVF process

  1. Ovarian stimulation (to grow multiple eggs) typically last from cycle day 2 to cycle day 12
  2. Trigger injection is typically done on cycle day 12
  3. Egg retrieval procedure is typically done on cycle day 14
  4. Laboratory fertilisation
  5. Embryo transfer into the uterus
  6. Pregnancy test 10–14 days later

A single IVF cycle from start to pregnancy test takes approximately 4-6 weeks. However, most women require preparation time before starting and may need multiple cycles.

  • Fresh embryo transfer: The embryo is transferred back into the uterus a few days after retrieval, within the same cycle.
  • Frozen embryo transfer (FET): Embryos are frozen and transferred in a later cycle.
FET may be recommended when:
  • The uterine lining needs more time to grow or stabilise
  • You experience Ovarian Hyper-stimulation Syndrome (OHSS) during stimulation
  • You want to give your body a rest after retrieval
  • You are planning genetic testing of embryosMany clinics now use FET as a preferred approach because it allows the body to return to a calmer hormonal state before transfer.

From a TCM perspective, the focus before a transfer is:

  • Improving blood flow to the uterus to support implantation
  • Regulating the menstrual cycle so the uterine lining grows smoothly
  • Supporting emotional steadiness to reduce cortisol and stress load
  • Calming the nervous system for deeper rest and repair

How TCM Prepares Your Body for IVF and IUI

TCM offers valuable support for assisted reproduction by optimising the internal conditions necessary for successful conception, implantation, and pregnancy maintenance. This is not about “quick fixes”, but helping the reproductive system function in a balanced and resilient way.

At Wo.men Matters, we strongly recommend to start preparing with TCM methods 3 months before your IVF cycle to increase the chances of success.

Why prepare 3 months prior to your IVF cycle?

Egg quality is one of the most critical factors determining IVF success. The eggs retrieved during your IVF cycle began their final development approximately 90 days earlier. Supporting your body during this critical window influences the quality of eggs that will be retrieved.

Even when AMH is low or FSH is elevated, the environment in which eggs mature can still be optimised. TCM promotes good egg quality through:

  • Reducing inflammation and oxidative stress
  • Regulating hormonal cycles
  • Improving blood nourishment

This might or might not lead to a reversal of AMH levels, but it does improves egg quality, which influences fertilisation, embryo grading, blastocyst formation and ultimately leading to a successful pregnancy.

Successful embryo implantation requires a receptive endometrium (uterine lining) with adequate thickness, appropriate structure, and good blood flow.

TCM supports endometrial development through:

  • Improving uterine blood flow
  • Nourshing Blood to build linig
  • Regulating oestrogen and progesterone
  • Warming the uterus to provide a warm, receptive environment needed for implantation

The stress of infertility and fertility treatment itself can interfere with reproductive success through multiple pathways. TCM helps promote calmer cycles by regulating stress pathways.

How TCM Supports IVF and IUI at Wo.men Matters

Pre-cycle preparation (3 months before):

Regular weekly acupuncture sessions, typically starting 2 to 3 weeks before stimulation, are crucial for optimising your body for treatment. These sessions work by regulating menstrual cycles, enhancing egg quality through improved blood flow and reduced stress, building endometrial thickness and quality, and naturally balancing hormones.

This is paired with our Traditional Chinese Medicine pills and syrup which strengthen the Kidney essence (constitutional reproductive energy), nourish Blood for egg quality and uterine lining, regulate Liver Qi to balance hormones and emotions, and strengthen Spleen to improve nutrient absorption and energy.

During stimulation phase:

Continued weekly acupuncture treatments support your body’s response to medications, reduce side effects such as bloating and mood changes, maintain emotional equilibrium during the demanding medication phase, and optimise follicle development.

Traditional Chinese Medicine is typically paused to avoid any interactions with fertility medications.

Around egg retrieval:

Acupuncture shortly before retrieval reduces anxiety, supports final egg maturation, and helps manage discomfort. Post-retrieval treatment addresses bloating, promotes recovery, and maintains uterine blood flow during the critical implantation window.

Embryo transfer support:

Acupuncture 1 day before the day of embryo transfer may improve pregnancy rates. Treatment immediately before and after transfer reduces uterine contractions, improves endometrial receptivity, enhances blood flow to the uterus, and calms the nervous system for optimal conditions.

During the 2-week wait:

Acupuncture during this anxious period supports early implantation, maintains progesterone support naturally, reduces stress and anxiety, and provides emotional support during the waiting period. We usually recommend conduct 3 acupuncture sessions within the first 5 days following embryo transfer, as this is the critical window for promoting implantation.

Combining TCM with medical treatment

We communicate with your fertility clinic when appropriate, respect your medical protocol timing and restrictions, adjust TCM treatment to complement not interfere with medications, and provide continuity of care throughout treatment cycles.

At Wo.men Matters, we specialise in integrating TCM with assisted reproductive technology, providing comprehensive support throughout your fertility treatment journey.

What we do differently:

Rather than generic “fertility support,” we provide individualised treatment based on your specific TCM pattern (Kidney deficiency, Blood deficiency, Qi stagnation, Cold, Damp, Heat, or combinations).

Your pattern is determined through comprehensive assessment including pulse diagnosis, tongue examination, menstrual history and characteristics, fertility test results, general health and symptoms, and emotional state.

Treatment addresses your unique presentation, not a one-size-fits-all approach. This personalisation is fundamental to our philosophy at Wo.men Matters.

At Wo.men Matters, our physicians are trained to understand and analyse fertility test results such as AMH, semen analysis, and ultrasound findings. We combine this with TCM pattern diagnosis, so your treatment plan is grounded in both medical clarity and holistic care.

What to expect during a TCM consultation with Wo.men Matters

Your first TCM consultation at Wo.men Matters typically lasts 30 to 45 minutes, allowing you to be fully heard.

This includes

Detailed Medical History

Current symptoms, menstrual history, medications, past treatments, lifestyle factors

Pattern Differentiation

Your energy levels, digestion, sleep, temperature preferences, emotional state

Physical Exam

Pulse diagnosis, tongue examination, and abdominal palpation if required

Frequently Asked Questions

Clear, practical answers to common questions about IUI, IVF, and TCM support.
IUI success rates on the first attempt depends on your age, diagnosis, and whether ovulation stimulation medications are used. Success is highest for women under 35 with unexplained infertility or mild male factor. The procedure works best when multiple factors align: adequate sperm quality, open fallopian tubes, good egg quality, and proper timing. Most pregnancies from IUI occur within the first 3 cycles if the treatment is going to work.
Most fertility specialists recommend trying 3 IUI cycles before reassessing strategy. If unsuccessful after this point, IVF offers substantially higher success rates and is generally more cost-effective than continuing multiple IUI cycles.

Yes, many couples try to conceive naturally or with less invasive treatments (IUI) before moving to IVF, unless there are clear indications that simpler approaches won’t work (such as blocked fallopian tubes or severe male factor). However, age considerations matter. Women under 35 can typically afford more time trying naturally or with IUI. Women over 37 may benefit from moving more quickly to IVF to maximise chances of conceiving before further age-related decline. This is especially critical for couples who wish to have 2 or more children. At Wo.men Matters, we support your conception journey naturally, regardless of the path you choose.

Whilst stress alone doesn’t cause infertility, chronic high stress from work can affect hormonal balance, cycle regularity, and potentially reduce fertility treatment success rates. This creates a vicious cycle: Stressful working environment potentially reduces the chance of conception, fertility struggles cause even more stress, which may make it even more difficult to conceive.

Real Stories and Holistic Healing

I have been doing IVF for 2 rounds without success. And there were no reason to the fail implantation, so I decided to try tcm, been going to TCM and visited Dr Koh for about close to 6 mth before next IVF session. Dr Koh is very patience and she will hear your concern and suggest accordingly the medication that suits you. I had acupuncture and Chinese medication during these times. The Chinese medication was at first making me too awake and Dr Koh adjusted accordingly. Post embryo insert I went additional acupuncture sessions. Now my baby boy is 1 month old, will visit Dr again when planning for 2nd child.

Visited Great wall TCM for fertility acupuncture while undergoing IVF. The service provided was remarkable; staff were able to set up appointments promptly for the period leading up to my transfer and there was minimal to zero waiting time. Was assigned to Physician Koh who is motherly and always thinking the best for her patients. Will recommend Great Wall TCM to anyone looking to increase fertility chances 🙂
